How do I make an appointment
You can make an appointment by calling our office at (386)230-4448. Additionally, you can use the self-scheduler under the "contact" tab to book your own appointment, please ensure registration paperwork is completed at least 48 hours before your appointment.
What are your fees?
We are more than happy to see clients who are covered by insurance as well as those who would prefer to pay out of pocket. Please check the insurance tab to see a list of insurance plans which we are in-network with. Call our office to for information about self pay rates.
Do you prescribe medications?
Yes, psychiatric nurse practitioners are licensed to prescribe medication.
When is your soonest appointment for a new patient intake?
Most new patients are able to be scheduled within one week as long as registration documents are completed and returned in a timely manner. For existing patients, same-day appointments are often available depending on urgency.
Do you have in person appointments available?
Yes, we offer both in person and virtual visits by appointment only. In an effort to maintain the privacy of our patients, we kindly ask that you only come to the office if you have an in person appointment scheduled as walk in appointments are not accepted. We are located in at
